Message in a bottle

The Message in a Bottle scheme is a simple idea designed to encourage people living on their own to keep their basic personal and medical details in a common location where it can be easily found in an emergency.

How does the scheme work?
As the name suggests, the personal information is kept in a bottle. The bottle is plastic, with a push fit lid and is big enough to hold the necessary information. The bottle is then kept in the fridge, where the emergency services will expect to look for it in the event of being called to your home. They will be alerted to your membership of the scheme by two labels which are unique to the scheme. One is fixed to the back of the front door  and the second is attached to the door of the fridge.

How much does the scheme cost?
Nothing! Lions cover the costs and will provide the bottles.

Where can I get a bottle from?
You can find them in a Lions box at your local surgery or chemists.

What information is needed?
The information you need to provide is quite simple, but you may like to ask a relative, neighbour or carer to help you complete the form.
The information required on the yellow form is:

 

1. Your full name, a very brief physical description, and your religion.

2. Your Doctor’s name and Surgery address and telephone number.

3. A brief description of any medical conditions you are suffering from.

4. The names, addresses and telephone numbers of two people who may be contacted in the event of a sudden illness or accident. 

5. Details of any pets which may be on the premises.

ALSO..............A passport sized photograph may be attached to the form, but this is only needed where there is more than one person living at the premises (to allow identification).

In addition, we strongly recommend that you also keep a copy of your repeat prescription list. This is produced every time a repeat prescription is ordered, and this information is of great value to the Ambulance Service.

Medic Alert / SOS Talisman
If you have a specific medical condition and routinely wear either a Medic Alert bracelet or necklace, or the similar SOS Talisman bracelet or necklace, you should continue to wear it.
